A ponytail is practically a summer uniform on Long Island. With the hair swept away from your face, the jawline becomes the focal point, often appearing sharper in some areas and softer in others.
In consultations, I've heard that this simple change in hairstyle prompted a closer look at the lower face. It can reveal how the skin and underlying tissue have changed with age, while offering a glimpse of the cleaner contour many patients hope to restore.
The ponytail test is an informal way to observe facial laxity. Pulling your hair back places a small amount of tension near the temples and sides of the face. Some people take it a step further by lifting the skin near their ears with their fingers.
This can help you identify what you’re drawn to, but it won’t predict the outcome of a facelift. A ponytail pulls the surface skin in a different direction than I would use during surgery. Pulling harder can also distort the cheeks or mouth, creating an effect that wouldn’t look natural.
Hair density can influence how noticeable the change feels. Thick hair may frame and cover more of the lower face when worn down, while fine or thin hair can leave more of the jawline exposed. Still, the test has less to do with hair type or ponytail circumference than it does with seeing the lower face without its usual frame.
A firm jawline depends on support from the skin and the deeper tissues beneath it. Collagen gives the skin strength, facial fat creates shape, and ligaments hold soft tissue in place. Each of these structures changes during the aging process.
As collagen production slows, the skin becomes thinner and less resilient. Facial fat may shrink in some areas and settle downward in others. The support beneath the cheeks can also weaken, allowing tissue to collect along the jawline. That’s where jowls come from.
Changes beneath the chin can add to the effect. Loose skin, excess fat, or separation within the neck muscles may blur the angle between the chin and neck. Some patients notice a small pocket beneath the chin, while others develop more visible sagging skin or what’s often called a turkey neck.
Genetics, sun exposure, weight changes, and skin quality all play a role. You can support your skin with sunscreen, a healthy diet, and consistent skin care, though none of us can stop age from changing the face.
Nonsurgical skin tightening can be a good option when the skin has begun to loosen, but the deeper facial tissues remain in a stable position. These treatments send controlled energy into deeper layers of the skin, prompting the body to create more collagen.
Results build gradually as your skin produces fresh collagen. Over time, you might notice firmer skin, smoother texture, and subtle refinement along your jawline. Downtime varies depending on the technology we use, but temporary redness, mild swelling, or tenderness are common.
I always tell my patients to be realistic about non-surgical options. While they can tighten mild laxity, they simply can't lift tissue that has shifted down into pronounced jowls. Depending on your goals, a series of initial treatments with periodic touch-ups usually works best to keep pace with natural aging.
Laser treatment may also help with wrinkles, sun damage, and uneven texture. Since laser procedures focus more on the skin’s surface, I may pair them with a skin-tightening treatment when both texture and firmness need attention.
When skin sagging and tissue shift become more noticeable, non-surgical options can only do so much. That’s usually when surgery becomes the most effective step. A facelift allows us to go deeper, repositioning the structural support beneath the surface to redefine your jawline naturally without that overly pulled look.
If you’re mainly noticing early jowling or a small amount of extra skin, a mini facelift might be all you need, but if moderate-to-severe laxity is spread across your lower face, a full lift can offer a complete refresh. We can also pair a facelift with a neck lift to smooth out muscle bands, remove excess fat, and clean up the profile under the chin.
I always view the lower face and neck as a single unit. Treating one without even considering the other can create an awkward contrast between your jaw and neck, but addressing both together keeps everything in balance, leaving behind seamless results.
Incision placement is another common question patients ask about. We place incisions carefully around the natural curves of the ear and within the hairline so they blend in as they heal. We will tailor the exact approach to your anatomy and goals.
Using your fingers to lift the skin can show that a firmer lower face changes your overall appearance. It can’t show how your cheeks, jawline, and neck will respond to surgery.
When I plan a facelift, I examine more than loose skin. Skin thickness, facial fat, chin projection, bone structure, neck anatomy, and previous cosmetic procedures all influence the plan. Two patients can perform the same ponytail test and need very different forms of treatment.
Good facelift results should still look like you. I want your expressions, facial shape, and individual features to remain familiar, with stronger support and a cleaner transition from the jawline into the neck.
Putting your hair up may be what first draws your attention to the lower face, but it doesn’t tell us which treatment you need. Different treatments are used to improve early skin laxity without surgery, while others have reached a point where skin tightening won’t create the degree of lift some patients are looking for.
